What to Expect From a Home Nursing Visit

From the first assessment to the daily care log — what a nurse actually does in your home, hour by hour.

If you have never arranged a nurse at home, it is natural to wonder what actually happens when they arrive. Here is what a good home nursing service looks like, from the first phone call to the daily routine.

The first call and the assessment

You describe the patient's condition, any recent hospital stay and what help you need. A coordinator then visits the home to see the patient and the surroundings, review the doctor's instructions and agree on a shift pattern — day, night or 24-hour.

What the nurse does on arrival

  • Reviews the care plan, prescriptions and the previous day's log.
  • Checks and records vitals such as blood pressure, pulse, temperature and blood sugar where required.
  • Sets out the day's medicines and confirms the schedule with the family.

During the shift

Clinical tasks follow the doctor's instructions: giving medication, dressing wounds, caring for catheters or feeding tubes, repositioning to prevent pressure sores, and helping with safe movement. Between tasks, the nurse watches for changes — a rising temperature, new swelling, poor appetite — and alerts the family or doctor early.

The daily care log

A written log records medicines given, vitals, meals, fluids, sleep and any concerns. It lets the family and visiting doctor see the whole picture, and it makes handovers between shifts reliable.

What a nurse does not do

A nurse follows the treating doctor's plan; they do not change prescriptions or diagnose on their own. In an emergency, the nurse calls the family, the doctor and, when needed, an ambulance.

How families can help

  • Keep prescriptions, reports and the doctor's phone number in one place.
  • Provide a clean space for supplies and a clear route to the patient's bed.
  • Share the patient's likes, routines and fears — comfort is part of care.
  • Ask for a supervisor's number and the log at any time.

Arisha Nursing Home matches a nurse to the patient's condition and keeps a supervisor reachable around the clock. This article is general information and does not replace medical advice.
